• Input your first name, last name & your birthdate
• Input the phone number associated on your vaccine card
• Create a 4-digit PIN

Once the information is all matched within the system, you will be redirected to a new page with a QR code. When scanned, the QR code will pull up your existing COVID vaccine record. You may also screenshot this code for future use.
