us this week is doctor Matthew Grunckemeier. We did not
have a chance to do this segment last week, so
it's our first real chance to talk about Dax Hill,
who unfortunately is done for the season with an ACL tear.

(00:45):
I want to ask you this in the immediate aftermath.
I'm talking like, while the the athlete is on the
field laying prone, perhaps is can you tell whether it's
the player is dealing with, in Dax's case, a tear
or a strain.

Speaker 2 (01:00):
Yes, when you're on the field and you're doing an evaluation,
you'll sometimes see the trainer or the team doctor get
down there, and they do want to assess before the
swelling sets up the knee and its stability. What the
ACL does is it stabilizes the knee so that the
shin bone can't slide too far forward on the thigh bone.

(01:23):
And you can see the doctors and trainers down there
examining a knee, and you can get a good exam
pretty quickly after an injury, before the adrenaline sets in
the swelling and the patient stiffens up, So you should
be able to typically tell a sprain versus a complete
tear from your field side physical examination.

Speaker 1 (01:44):
On a typical for a typical injury like this, and
not that any injury is typical, but usually how soon
is surgery performed after this sort of injury.

Speaker 2 (01:56):
Well, it does kind of depend on the amount of
swelling and the preoperative range of motion, but probably within
a couple of weeks.

Speaker 1 (02:05):
Are there things that could complicate his recovery post surgery?

Speaker 2 (02:11):
Oh, sure, stiffness is of concern, lack of you know,
muscle strength, rehabilitating the knee is really important. A lot
of physical therapy going to be involved.

Speaker 1 (02:23):
This might be a dumb question, not the first one
that I've asked. If the Bengals do what we all wish,
which is make a deep playoff run, is there any
any chance at all that we could even cling to
the idea that he could come back and play.

Speaker 2 (02:39):
I would say not. You're looking at a minimum of
six months maybe longer to get safe to turn to play.
So I would say probably count out this season, although
I've been surprised before some athletes will bounce back sooner
and be looking for next season.

Speaker 1 (02:57):
In the same game where Daxhill got hurt, an offensive
lineman named Jackson Kirkland also suffered an injury a bicep
muscle taror. Whenever I hear this sort of injury that
just sounds and I'm not sure why relative to other injuries,
that just sounds painful. Walk us through the process of
evaluating and then ultimately diagnosing this sort of injury.

Speaker 2 (03:17):
Yeah, So, if the muscle is torn, it's going to
be like other muscle injuries, something's going to be treated
with time and physical therapy, not typically surgical. If the
tendon tears off the bone, that's a different story, and
there may be surgery involved in a more prolonged rehabilitation time.

Speaker 1 (03:39):
All right, I also want to ask you about Aiden Hutchinson, who's,
you know, arguably right now this season at least been
the best player, best defensive player in football. They're playing
Dallas on Sunday and he suffers one of those injuries
that the TV network decides were only showing once. And
I saw it and it was gruesome, and I'm glad
they didn't show it again. So he breaks his tibia,

(04:02):
and we hear tibia, we hear fibula again. Maybe from
my standpoint, kind of a dumb question, can you clarify
what the tibbi and phibula are?

Speaker 2 (04:11):
Sure the tibia is the larger of the two leg bones,
the shin bones, so to speak, and the fibula is
the smaller strut. When they both fracture, it's obviously a
highly unstable injury, and you know, you can see the
leg moving around in ways that are quite unnatural. So
I think that's one of the reasons why they are

(04:33):
only going to show that replay once.

Speaker 1 (04:35):
Yeah, I'm glad they did. They perform surgery immediately after
he suffered the injury, and the lines were on the
road in Dallas. Why is it so critical with an
injury like this to get him on the operating table asap.

Speaker 2 (04:49):
Well, you know, you can get a lot of associated
soft tissue swelling, and because your your tibia or shinbone
is so close to the skin, any type of skin
tear could lead to severe complications, could lead to excessive swelling,
which is known as compartment syndrome. So I think just

(05:09):
in order to prevent those potential complications, surgery is recommended
in a timely fashion. Doesn't have to be the same day,
but should be probably within the first few days.

Speaker 1 (05:21):
Is the fact that they were on the road or
we're going to have to travel back to Detroit. Is
that a factor in determining that they had to do
the surgery immediately?

Speaker 2 (05:29):
I would imagine so. And just the pain level, you know,
with such an unstable injury in a splint, it's just
not going to probably provide very much comfort for the
athlete as they're trying to travel, whether that involves planes
or whatever way they're going to travel, crutches. It's probably

(05:52):
just way simpler and a less painful route for him
to just go ahead and have the surgery, get the
bones stabilized, usually through a rod that goes through the
center of the tibia, and that's going to be the
surgical treatment there.

Speaker 1 (06:08):
That injury was tough to watch, and yet you hear
about the surgery, and then he's on a team that
has a chance to play deep into the postseason. There
are reports that if the Lions made it all the
way to the Super Bowl, perhaps he could come back
and play. That seems almost improbable. Is it as improbable
as I believe?

Speaker 2 (06:28):
Pretty improbable? You're talking twelve weeks for the bone to heel,
but then you got muscle wasting. I wouldn't count on it.
