weeks later they flunk it. Theyget a stress test, they flunk it,

they get a heart gath, theyget bypass surgery, and they say
that you and Tony and Dave helpsave their life. And so God bless
you all for doing this. Thisis preventive cardiology. We're talking about it.
Well, listen, and I rememberthat we had one listener go in,
thought everything was fine, wasn't overweight. He wound up having three out
of the four with major clogs.Obviously won't name him names because of the

hippopotamus oath Sey docs. But Iwant to talk about what you do we
would call preventive cardiology. Yes,and and I'm grateful that I found out
about this. Uh. I foundout through Terry Miners. Listened to the
Terry Miners show. You were aguest, and they eventually came on here.

And then I got mine done.I was always scared, scared to
death because in my eyes and inmy brain, your heart's a ticking time
bomb. You just don't know.And I was every time I felt the
pain and I started to get older, is it going to be a heart
attack? What's going on with me? I got mine done and I had
peace of mind, and thank god, everything was fine. But talk a

little bit about preventive cardiology. Sowhat we're trying to do here is find
out if you and there's a lotof different heart problems, we're talking about
the number one killer of men andwomen coronary artery disease. So preventive cardiology
seeks to identify people that have cornaryartery plaque hardening of the arteries the other

term, early in life so theycan do the right things. They can
be motivated to exercise, eat theproper diet, take a stating to slow
down plaque build up, but mostimportantly, to be aware that, yes,
I have cornary art disease. I'mraising my hand right now because I
do have some product. I haveno symptoms, but I know about it.
The biggest thing is if I havechest pain tonight at nine o'clock,

I will not sit at home andthink it's indigestion. I will call nine
one one right to the hospital.So that's the advantage of knowing. In
the test, we always talk aboutthat you've had I've had, Terry's had,
Dave's had uh Tony's had the cornerycalcium scan. Now, as a
segue to that, I was alsogoing to segue about Birno's Pizza being voted

have a special guest today and sheand her husband are both dear friends of
mine. They came to Louisville towork here and to work in preventive cardiology.
And her husband's an invasive cardiologist,doctor UVA evasive cardi invasion that means

you don't want to talk to him, but he'll come in your office and
talk to you no matter what aboutright, he'll put the cather in through
your wrist. And but we havedoctor Myron Mrin Shetty s ah G t
t Y who just joined University LouisvelleHealth and University Academic Teaching as a faculty
member last fall. Now let mebrag on her. Please, just trained
in Chicago for her cardiology fellowship.But then she went to Colombia New York

Presbyterian in Manhattan to do a specialyear of non invasive imaging and CT coronary
calci CT cornary angiography, which isa fancy calcium scan with an injection to
look for the soft plaque. Nowthe cornery calcium scan that I do is
a ninety nine dollars test. It'sa basic test just to see where you

are on the continuum. Then dependingon symptoms, you need further testing.
If you're asymptomatic and you have alow score, you may not. But
doctor Shetty studied and is an experton CT cornary angiography. So without further
Ado Dwight, let me shut upand let you to wonderful people talk.
Good morning, doctor Shetty, howyou doing. It's great to meet you.

And you're also the director of theWomen's Health Programs University of Louisville.
Correct, Okay, let me jumpoff of that. I want to talk
about the difference between the scan we'retalking with him and the scan with you.
But first I want to talk tothe women for a second. Because
things get labels put on them.Heart attacks is one of them. For
whatever reason, it seems like that'sa male problem that all know it happens

to males. Women have heart attackstoo. Talk about the risk for women
if you don't mind, absolutely,that's a great question, and that's a
huge misconception that heart disease is justabout men. But the fact is that
one in three women is going todie from a heart attack. We focus
on you know, the bikini areasfor women, breast cancer screening o bg

YN screening, but one in twentyfive will die from breast cancer, whereas
one in three will die from heartdisease. Specific to women, one in
three ye will die in heart disease. It's a huge number, which is
why it's so important to spread awarenessabout women. And the thing with women
is women may present with chest painor you know, that sensation of an

(05:53):
elephant sitting on their chest. Buta lot of times women have atypical symptoms,
especially if they're older or they havediabet things like feeling nauseous or indigestion,
that sense of impending doom that couldbe a precursor for a heart attack.
Really yes, sometimes just discomfort inthe shoulder blades that goes down your
arm. So very atypical symptoms,so it's much harder to detect. But

(06:17):
that's why we want to raise awarenessfor women. So talk about the difference
between the calcium scores can that doctorSadlowe does and then what you do,
because it's very interesting, is itchecks for a different type of plaque as
well. Is that right? Yes? Exactly. So if you've had high
cholesterol for a very long time,that's going to start laying down in the

blood vessels of your heart. It'ssoft and mushy, what we call soft
plaque. The body then goes andtries and calcifies that, so that's calcified
plaque. With a calcium score,what we can see is that calcified plaque,
but earlier, one step before thatis the soft, mushy plaque.
So first you can get a calciumscore to see if you even have cornio

archy disease if you're between the agesof forty to seventy, but if you're
having symptoms of chest discomfort, insteadof jumping to a cardiac cat straight off
before what we would do is astress test. Okay, but the stress
test doesn't inform you to say whetheryou have plaque build up already. It
only tells you if it's obstructive.What if you have plaque build up,
you want to prevent going to thestage where you have a heart attack,

(07:23):
And that's what a ct corny andigramdoes if done correctly, it's about sixty
ccs of contrast die and it's prettyquick. You give get a tablet of
metropolol to lower your heart rate.Go in, get the scan, not
painful at all, and then wesee beautifully each in every blood vessel of
your heart. Beautiful. Okay,let's go back to my friend doctor Henry

said, Loo, I want totalk about it gave me peace of mind
getting in the calcium cornary. Yeah. But with that said, behavior goes
on and on and on and on. When am I due for the next
one? After you get one?And I think I scored like a fifteen
And you said that I had theheart of a superhero and that any other

(08:09):
man might have died on the spot. But thankfully as strong as I am
and my heart was in such bettershape and superhuman that I was good.
But with that said, whence shouldyou get your next one? You get
one, you get clear, oryou have something done. Is I guess
it's not as arbitrary on you howyour heart health is. But in my
case or someone else's, when doyou get the next scan? Well,

(08:30):
first of all, the best thingthat you had when you got your scan
is you were able to show yourwife Susan, that you had a heart
right, you had very mild earlyplaque. So but if you didn't know
that, you may not be ona stanton, or you may not be
watching your diet. You may notbe excising. So having said that,
there's one group that says, onceyou get a scan, you don't need

(08:52):
another one. I don't necessarily believein that. Now, once you have
advanced plaque, let's tell the audiencewhat we're tough about. A simple,
non invasive You just lay on acat scan table for this ninety nine dollars
out of pocket test. There's noivy. It takes two minutes and then
it and then the computer generates ascore. So if you get a zero,
that's the only normal score. Ifyou get a score from one to

(09:16):
ten, it's called minimal calcified plaque. Eleven to one hundred is mild plaque.
So that's you were in the earlycategory. I was in the moderate
that's one hundred to three ninety nine. Tony was at the upper end of
that. Tony and I are inthe same group. Over four hundred is
advanced. So I have the lowestnumber. By the way, Oh,
what was your number day? Itwas like five or so. Oh that's

(09:37):
good, rock On. So backto your original question, it's reasonable to
consider a follow up in five years. Now, what that did for me,
I had a five year follow up. There are experts of Johns.
Hopkins was saying I didn't need it, But but what it did for me.
My score went from forty eight whenI was fifty five to two seventies.
So that caught my attention. Anduh, you know, and I

(09:58):
know, you know, maybe someof the soft plat cut cows fied.
But anyway, what that motivated meto do is to double my statin dose,
to work on try to eat better, eat salmon two or three times
a week, reduce cow. Butalso I doubled my statin dose and I
doubled my aerobic exercise with the walls. Yeah, and you're constantly doing that,
and that's fantastic because you get thesegroups together. So for you,

(10:20):
five years would be reasonable to consider. I don't force patients to have a
follow up, but I tell themthis is these are the pros and the
cons. But for me, itwas a great motivator. Now, if
you smoke cigarettes, try to quit. But if you're a cigarette smoker or
a brittle diabetic, you might wantto consider a follow up in three years,
especially if you get a zero scoreor if you're not on a statin,

(10:41):
because this test probably the most helpfulthing is to help a doctor tell
a patient, a general medical doctortele a patient whether they should be on
a statin or not. More withdoctor Shetty here in just one second,
I got another question or comment foryou, and you were talking about behavioral
what you eat? Did you smoke? Man? I ate horribly. I
smoked and still drink. My behaviorwith heart attack one oh one. I

(11:07):
lost seventy pounds of fat. Seventypounds, okay, and that's before that
was after I lost the seventy pounds, and then after that I wind up
getting this calcium score. So Igo in there and I think, man,
I'm gonna have the worst of theworst. Look at look at all
of the fried foods, look atall the fast foods. You've eaten,
your cigarette, smoking, the works. I go in and I get it

(11:30):
fifteen. Now you and I knowguys that their skin is a rail.
You think they're the picture of health. And they go in arbitrarily, Yeah,
they all get it done. Docsad this big deal. They go
in and they get a horrible score. It's not it's not always things are
not always what they seem doc andthat's why you get the test. Now.
For example, this Saturday, ourgroup walked Cherokee Park Loop at ten

thirty we had fourteen people. Thetwo fastest guys walking a fifteen minute per
mile pace. We'll call them Fredand Bill because that's what Brian Stanford called
it when we were But they bothhave had bypass surgery. If you look
at him, they don't have anextra pound. They look as fit as
can be. Bad genetics, soif you've got a family history, that's

(12:13):
one of the number one reasons toconsider a cornery calcium skin if you're at
appropriate age. We used to saywomen at fifty five, men at forty
five at State of Kentucky. Wemissed too many people, so now we're
dropping men down to thirty five inselect cases, especially diabet extract and women
to forty. So we've changed thesethings, and nothing's written in stone.
There might be an exception to therule, but anyway, that's a generalization.

(12:35):
So I would say five years foryou, since you don't smoke anymore,
you've lost a ton of weight,you're exercising, you do a great
job, and five years out youlook at and see what your score is
side whether maybe we work a littleharder on this. Maybe we go up
on your stand and does doctor Young, maybe he'll go doctor Scott Young,
my doctor. But just to kindof recap, listen up, loved ones

(12:56):
on this. It doesn't matter whatyou look like. You could be in
the best shape is you can havezero let's ridiculous, ten percent body fat.
You could still be in danger justsimply because of genetics. You could
have eaten, say the proper thingsyour entire life. Exercise, just find
out, if not for you,find out for the ones that love you.

(13:16):
If I shift again over here todoctor Shetty, Doctor Sheddy, you
had mentioned that women have different indicatorsof heart attack. I find that fascinating.
Why is that? And could yougo over those symptoms again one more
time? Absolutely so, they saywomen's hearts are more fragile, and there
is some truth to that because womenare not small men. Our hearts are

just built different. So the caliberof the blood vessels that supply our herd
or smaller, and the walls ofour heart are thinner than men. So
the risk of complication is higher.And obviously because we have different types of
hearts. We manifest these differently.And just to reiterate things like, you
know, a sense of impending doom, nausea in the that doesn't go away,

of fatigue that goes on for acouple of weeks. Don't ignore those
symptoms discomfort and your shoulder blade goingdown your arm. If you have any
of those, seek out of cardiologists. So we're talking to men and women.

people visualize this as a male problem. It's a female problem too, in

but I mean, I've just gottenback from Scotland where we walked all
over the place, climed a bunchof stars. Didn't have any real issues.
But talking to my doctor and listeningto doctor Sadloe on the radio,
I asked if she would refer mefor a cardiac calcium scan, which she
did, and it was twelve hundredand fifty Wow, wow in the advance.

Within a week I was in thecardiologist office, and two weeks later
I was getting a cardiac cat andthey saw it eighty five percent blocking jam
lad which is the widow maker,and they went in and repaired it.
So I'm very great. Congratulations.Yeah, yeah, thank god, Dan,

what a story man. And youhad no idea, no idea,
no idea. I had some indigestionand occasionally I would have you know,
if I was like working out orsomething, I'd have a little bit of
just pain, but always considered itwas more like pleurisy or something, you
know, because I take heat breastand it would eventually go away. So

uh yeah, no real, noreal big symptoms. Just uh I hadn't
had that best, I might notbe here today. And then those kind
of stories are what make me,what makes me want to keep working at
the ripe old age of Medicare AIDS. So that's a great story. And

(26:32):
yeah, Dan, thank god youpaid attention. Yeah, thank god that
you got the calcium score scan done. And and Dan, thank you so

don't take it as a fear tactic. This is not a paid endorsement or

And the statistics show if you geta zero, the chance of soft plaque
that doesn't show up on these ninetynine dollars tests is only about four four

absolutely is. If you have establishedcorney archery disease, if you've had a
stent in the past, if you'vehad bypass surgery in the past, those
are the patients you already know thediagnosis, so you wouldn't get the calcium
score. Also, for people whoare below the age of thirty five sometimes

forty, they may or may nothave the soft black, but it hasn't
been enough time for them to calcifytheir black. So those are the ones
we avoid. And lastly, ifyou're one of those types of people who
have anxiety in terms of their health, then this will probably accelerate that if
you see a score. But ofcourse, if you take it in the

right spirit and use it as amotivator, it can be a huge lifestyle
change, just like your example.All right, let's go to Richard at

symptoms, and you've had two bypasses, and I know the doctors that you're
seeing are telling you things to do. You bring up a very important point.
You had shortness of breath. Wehave one of live hikers that never
had a single chest pain but gota Castling score over five thousand, and
he had bypass surgery and he's doinggreat now. So it's not just chest

pain, it's shortness of breath,unexplained shortness of breath and also unexplained fatigue.
Have some patience. No chest paintershort and says you just don't feel
right. They feel fatigued, andyou do a calcium score it's two thousand
and well you kind of found theright spot. Well, listen, Richard,
I'm certainly sorry for your loss andno parents should ever have to go
